Job Function:

Provides and directs PhysicalTherapy services to residents, including assessment, treatment,program planning and implementation, related documentation, andcommunication. Functions under physicians’ orders.

ResponsibilitiesInclude:

• Provides Physical Therapy services to residents andinitiates referrals when appropriate and in accordance withdepartment procedures.
• Conduct screenings of residents at regularintervals to determine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatment planningand implementation.
• Conduct specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implementsand conducts treatment as outlined in treatment plan.
• Followsmanagement direction.
• Performs other duties as assigned.

Education and Experience:

• Bachelor of Science in Physical Therapyfrom an accredited program.
• Successful completion of NationalCertification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
