Position Summary:
The PT – Physical Therapistprovides direct rehab care in physical therapy to patients uponphysician referral in accordance with all applicable laws,regulations, and Life Care standards.
Education, Experience, andLicensure Requirements:
- Graduate of an accredited program inphysical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPTor DPT)
- Currently registered/licensed in applicable State. Mustmaintain an active license in good standing throughout employment.
- One (1) year experience in post acute care or related settingpreferred
- CPR certification upon hire or obtain duringorientation.
CPR certification must remain current duringemployment.
Specific Job Requirements:
- Participates in communityhealth matters/projects as appropriate
- Proficient in MicrosoftWord, Excel, and e mail
- Liaisons with patients, families, supportdepartments, etc., to adequately plan for patient needs
- Mustdemonstrate good body mechanics at all times
- Make independentdecisions when circumstances warrant such action
- Knowledgeable ofpractices and procedures as well as the laws, regulations, andguidelines governing functions in the post acute care facility
- Implement and interpret the programs, goals, objectives, policies,and procedures of the department
- Perform proficiently in allcompetency areas including but not limited to: patient rights, andsafety and sanitation
- Maintains professional working relationshipswith all associates, vendors, etc.
- Maintains confidentiality ofall proprietary and/or confidential information
- Understand andfollow company policies including harassment and complianceprocedures
- Displays integrity and professionalism by adhering toLife Cares Code of Conduct and completes mandatory Code of Conductand other appropriate compliance training
Essential Functions:
- Follow physical therapy treatment plans for patients
- Establish,assess, and modify realistic, measurable, timely, and functionalgoals
- Oversee and evaluate care given by PTAs, Rehab Aides, and PTand PTA students
- Chart appropriately and timely
- Utilize therapysoftware appropriately and accurately
- Exhibit excellent customerservice and a positive attitude towards patients
- Assist in theevacuation of patients
- Demonstrate dependable, regular attendance
- Concentrate and use reasoning skills and good judgment
- Communicateand function productively on an interdisciplinary team
- Sit, stand,bend, lift, push, pull, stoop, walk, reach, and move intermittentlyduring working hours
