HolidaysPosition Summary:

The PT -Physical Therapist provides direct rehab care in physical therapyto patients upon physician referral in accordance with allapplicable laws, regulations, and Life Care standards.

Education,Experience, and Licensure Requirements:
  • Graduate of an accreditedprogram in physical therapy, BSPT, MSPT, or DPT (2003 or later musthave MSPT or DPT)
  • Currently registered/licensed in applicableState. Must maintain an active license in good standing throughoutemployment.
  • One (1) year experience in post acute care or relatedsetting preferred
  • CPR certification upon hire or obtain duringorientation.

CPR certification must remain current duringemployment.

Specific Job Requirements:
  • Participates in communityhealth matters/projects as appropriate
  • Proficient in MicrosoftWord, Excel, and e mail
  • Liaisons with patients, families, supportdepartments, etc., to adequately plan for patient needs
  • Mustdemonstrate good body mechanics at all times
  • Make independentdecisions when circumstances warrant such action
  • Knowledgeable ofpractices and procedures as well as the laws, regulations, andguidelines governing functions in the post acute care facility
  • Implement and interpret the programs, goals, objectives, policies,and procedures of the department
  • Perform proficiently in allcompetency areas including but not limited to: patient rights, andsafety and sanitation
  • Maintains professional working relationshipswith all associates, vendors, etc.
  • Maintains confidentiality ofall proprietary and/or confidential information
  • Understand andfollow company policies including harassment and complianceprocedures
  • Displays integrity and professionalism by adhering toLife Cares Code of Conduct and completes mandatory Code of Conductand other appropriate compliance training
Essential Functions:
  • Follow physical therapy treatment plans for patients
  • Establish,assess, and modify realistic, measurable, timely, and functionalgoals
  • Oversee and evaluate care given by PTAs, Rehab Aides, and PTand PTA students
  • Chart appropriately and timely
  • Utilize therapysoftware appropriately and accurately
  • Exhibit excellent customerservice and a positive attitude towards patients
  • Assist in theevacuation of patients
  • Demonstrate dependable, regular attendance
  • Concentrate and use reasoning skills and good judgment
  • Communicateand function productively on an interdisciplinary team
  • Sit, stand,bend, lift, push, pull, stoop, walk, reach, and move intermittentlyduring working hours
