Job Description As a Physical Therapist, you will serve as a key partner in eachpatient’s plan of care, working directly in patients’ homes acrossSt. Peterburg & Clearwater, FL and surrounding areas. You will playa vital role in restoring mobility, promoting independence, andensuring patient safety within the home environment. Your expertisewill be used to maximize quality of life and movement potentialthrough assessment, intervention, and ongoing evaluation. You will collaborate closely with physicians, caregivers, and thebroader clinical team to develop and implement individualizedtreatment plans. This position is ideal for professionalspassionate about making a tangible difference in the lives ofpatients by supporting their recovery and empowering them to livesafely and independently at home. Duties and Responsibilities Identify and meet patients’ goals andneeds through comprehensive assessment and intervention. Developand implement individualized care plans using a variety oftreatment techniques. Deliver quality, evidence-based, andpersonalized care to patients in their homes. Motivate and supportpatients during therapy to help them achieve optimal function.

Promote healthy lifestyles by improving patients’ strength,flexibility, balance, and coordination. Evaluate treatment effects,monitor progress, and communicate updates to the care team.

Collaborate with physicians, caregivers, and other healthcareprofessionals. Maintain accurate and timely documentation ofpatient care. Other responsibilities as assigned. Required Experience/Skills Minimum of 1 year of skilled physicaltherapy experience. Proven work experience as a physical therapist,ideally in a home health or clinical setting. Licensed/registeredas a physical therapist in the state of Florida. Ability to managepatients with varying personalities and needs. Current knowledge ofphysical therapy treatment practices. Excellent interpersonal andcommunication skills. Demonstrated commitment to ongoingprofessional growth and learning. Professional behavior and strongorganizational skills. Nice-to-Haves Experience working in home health care. Familiaritywith electronic health records or home health documentationsystems. Interest in patient education and community healthpromotion. Education Bachelor’s degree from an accredited Physical Therapy program(minimum requirement).
