A Few Must-haves for

This Position:

Current licensure as a Physical Therapist in the state ofMinnesota. In the last five (5) years, two (2) years of experience as aPhysical Therapist that is relevant to the patient populationassigned preferred. Able to travel within geographic area serviced by office. validdrivers license, and proof of auto insurance is required. CPR certificationSome functions of what you will do as a

Physical Therapist:

Assists the Physician or other licensed practitioner inevaluating the level of patient function and performance andequipment needs as well as in developing the resulting plan ofcare/treatment including appropriate long and short-term treatmentgoals, selection of interventions, and the intensity, frequency andduration of services to meet the needs of an individualpatient. Completes initial and ongoing comprehensive assessments atappropriate time points as assigned. Participates in ongoing interdisciplinary assessment of thepatient. Participates in the development and evaluation of the plan ofcare in partnership with the patient, representative (if any), andcaregiver(s). Provides services that are ordered by the physician asindicated in the plan of care. Provides interventions and therapeutic treatment in accordancewith the physician certified plan of care and physical therapyprofessional standards of practice.

• Regularly assesses changes in the patient’s status since thelast visit/treatment day, and determines whether the plannedservices should be modified, performing a re-evaluation andobtaining revision orders from the Physician as indicated.
