Position Summary:

We are currently seeking a Per Diem Physical Therapist (PT) to join our in-house rehabilitation team. As a PT, you will assess, plan, and provide physical therapy services to patients recovering from illness, injury, or surgery, with the goal of maximizing function and quality of life.

Key Responsibilities:

Evaluate patients and develop individualized treatment plans based on clinical findings and functional goals. Provide direct physical therapy services to improve mobility, strength, balance, and overall physical function. Collaborate with Physical Therapist Assistants (PTAs) and other members of the rehab and nursing teams to coordinate care. Monitor and document patient progress accurately and timely in accordance with regulatory and facility standards. Educate patients, families, and caregivers on treatment plans, home exercises, and safety techniques. Ensure compliance with all state, federal, and facility regulations.

Qualifications:

Graduate of an accredited Physical Therapy program. Current licensure as a Physical Therapist in the state of Florida. Experience in a skilled nursing or subacute rehab setting preferred but not required. Strong clinical reasoning, documentation, and communication skills.
