As thePhysical Therapist, you will: Evaluate and assess patient to determine their appropriatenessfor care Utilize the available tools and resources to developpatient-specific care plans with the appropriate level andfrequency of care Communicate with the care team, physician, payors, patients,and families to address care needs and fulfill the patient’s careplan Deliver high-quality skilled care and supervise the care ofother care team members Thoroughly document care delivery daily in our EMR system Contribute to a culture of caring through individualaccountability and teamwork Skills for Success Compassionate in care delivery, focused on results Solution-driven, self-motivated, and responds with urgency Love learning, motivating and inspiring people Enthusiastic about working to the highest level of PTlicense Familiar and comfortable with technology.
