ResponsibilitiesResponsible for helping patientsmaintain, restore and improve movement, activity andfunction.
Provides interventions, conducts reexaminations (every 30days, at minimum), modifies interventions as necessary, andperforms discharge planning.

QualificationsCurrent license to practice physical therapyspecific to the state you’re assigned to work in.
Graduate from anaccredited physical therapy educational program as set by theCommission on Accreditation of Physical Therapy Education(CAPTE).One year related physical therapy experience for PTsentering home health care to ensure that knowledge and skills aresufficient to safely provide physical therapy services topatients.
If less than one year of experience, approval fromregional clinical leaders is required as well as appropriatementoring and/or participating in a residency program.
Current CPRcertification.
