JOB PURPOSE

To provide evaluative, consultative and treatment interventions toall home health patients in their place of residence referred by alicensed physician and will maintain effective communication withhome health partners patients, patient’s families and otherinternal and external customers as appropriate.

KEY RESPONSIBILITIES
  • Evaluate the patient’s level of function and skilledtherapy needs.
  • Treat the patient to relieve pain, develop or restorefunction, and obtain maximum outcomes according to the establishedplan of care.
  • Advise and consult the family and other interdisciplinaryteam members to promote optimal functional outcomes.
  • Assesses, monitors, documents and reports progress ofpatient’s health and condition using required documents viaclinician preferred technology, and within establishedtimelines
  • Helps maintain continuity of care by collaborating withthe interdisciplinary team and attending physician to develop,integrate, and manage the patient care plan of care, makingreferrals to other disciplines as needed
  • Demonstrates intermediate knowledge of home care andcompetency in discipline for specific patient care skills, requiredfor the provision of care
  • Conducts field visits to (1) evaluate & initiate start ofcare and (2) conduct clinical outcomes monitoring, follow-up andcare performance improvement initiatives
  • Utilizes accepted company teaching materials inpatient/family education
  • Completes documentation timely, accurately, and at thepoint of care, according to industry standards (e.g., OASISassessments, SOC/Admissions, Recertifications, ABN’s, subsequentvisits, physician orders, care coordination, etc.)
  • Meets agency productivity standards and utilizes time andresources effectively and efficiently
  • Participates in center/agency surveys (Licensure/JointCommission), mandatory in-services, case conferences, staffmeetings, and programs (e.g., Performance Improvement Program) asrequired
  • Follow established safety regulations and procedures, andreport job-related functions/tasks that involve occupationalhazards, as necessary
  • Maintains current license, certification, and clinicalrecords to meet federal, state and agency regulations andguidelines
  • Recognizes and responds to priorities in patient careshowing initiative and sound judgment.
  • Demonstrates willingness to adapt to change in workloadand assists with training/orienting others.
  • Accepts responsibility to report any alleged or suspectedviolations to supervisor and/or others until the issue isresolved.
  • Ability to be self-directed, prioritize, manage timeappropriately, and meet outcomes and strategic goals by targeteddeadlines. LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS
  • Must have a valid professional license in State ofpractice.
  • Graduate of an accredited Physical Therapy Program.
  • If foreign-trained, the program must be deemedsubstantially equivalent to a Physical Therapist entry leveleducation in the U.

S. by a credentials evaluation organizationapproved by the

APTA. PREFERRED SKILLS AND EXPERIENCE
  • Prior Home Health experience ispreferable.
