The physicaltherapist assesses, plans, organizes and participates inrehabilitative programs that improve mobility, relieve pain,increase strength and decrease or prevent deformity of patientssuffering from disease or injury. Services are provided across thefull continuum and rendered in a cost conscious, quality focusedand customer oriented manner. Compliance with all applicableregulatory standards is also required in this role.
CoreResponsibilities and Essential Functions:
Patient Assessment Incollaboration with the physician, responsible for the establishmentand ongoing evaluation of treatment programs, plan of care, goalsand discharge plans for the patients as related to the physicaltherapy needs of the patient. Establishes a plan of care inconjunction with the patient and/or family members. Establishesrealistic discharge plan incorporating patient and/or family membergoals. Treatment Planning and Provision of Care Implements thephysical therapy treatment plan Selects appropriate treatmentactivities to progress patient towards goals. Revises goals/plan ofcare with patient/family input Provides ongoing patient/familyeducation and training Provides discharge instructions, follow-upand referral to community resources as appropriate Acts as areferral source for staff, families, physicians on services andequipment related to rehabilitation services. DocumentationDocuments the evaluation and plan of care according to departmentalguidelines Documents each treatment session according todepartmental protocols Completes and submits family education,discharge and other required documentation within facility andprogram guidelines General requirements Adheres to ethical businesspractices by striving to perform in a manner that conforms to thehighest standards of ethical behavior, integrity and honesty.
