THIS POSITION MAY BE ELIGIBLE FOR UP TO A

$10,000 SIGN-ONBONUS (restrictions may apply) •The Physical Therapist providescomprehensive professional physical therapy service in accordancewith the mission and objectives of Nascentia Health. Although thisposition primarily serves Onondaga County, visits to surroundingcounties may be required as needed.

Performance Responsibilities andStandards:

1. Ensure quality and safe delivery of home healthservices.

Provide safe Physical Therapy interventions toclients/families in accordance with agency policies andprocedures.

Provide client assessments using best clinicalpractice.

Develop appropriate plan of care.

Maintain client carerecords using approved documentation and format.

Documentationreflects care and interactions.

Documentation is completed in atimely manner.

OASIS documentation is completed within regulatorytimeframes.

Documentation is clear & concise.

Follow professionalcode of ethics and standards of practice.

Manage a diverse clientcaseload.

Communicate client care issues to interdisciplinary team2.Assume responsibility for supervision of para professionalsrelevant to patient care planMonitor job performance of home healthaides and delivery of home health aide services3. Comply withNascentia Health policies and procedures.

Maintain Nascentia Healthproductivity standards.

Maintain adherence to agency policies andprocedures including Corporate Compliance, Ethical Standards, andConfidentiality (HIPAA).Demonstrate knowledge of federal, state andlocal government rules and regulations.

Demonstrate knowledge anduse of infection control practices.

Adhere to uniform policy.

Adhereto quality improvement standards.

Keep personnel requirements up todate, i.e. health requirements, professional license requirement,and other Nascentia Health requirements.

Demonstrate skillscompetency through periodic evaluations.4. Participate in personalgrowth and development.

Participate in education in-services andmeetings.

Maintain and update clinical knowledge on an ongoingbasis.5. Present a professional image to clients and thecommunity.6. Perform all other job duties asassigned.

Requirements:

Currently registered by New York State andthe American Physical Therapy Association and educational standardboard as applicableCurrent New York State Physical Therapylicense.

Current New York State driver’s license and automobileinsurance.

One-year experience as a therapist preferred.

PhysicalRequirements:

Ability to sit or stand at a desk 90% of thedayAbility to perform tasks involving physical activity (includingbut not limited to): Pushing/pulling on average 50 lbs with a maxof 150 lbs; lifting/carrying on average 25 lbs with a max of 40lbs; patient transfers, extensive bending, standing, kneeling,walking in various weather conditions and ability to go up and downstairs.

Ability to drive in ALL weather conditions (rain, sleet,snow, wind).Visual/hearing ability sufficient to comprehend writtenor verbal communicationAbility to use telephone and communicateover the phoneFrequently uses a laptop computer to enter orders andobtain product informationBloodborne Pathogens Exposure Category I- Employee has tasks who routinely involve exposure or contact withblood and body fluids.
