Physical Therapist Advanced Home Rehabilitation Norcross, GA 30093 At Advanced Home Rehabilitation, we are dedicated to providing high quality clinic-based and Assisted Living Facility (ALF)/Independent Living Facility-based therapy services. We proudly serve a mostly geriatric population, within our clinic and within an ALF, focusing on improving our patients’ quality of life, independence, and safety. We are seeking a compassionate and skilled Physical Therapist to join our exceptional team and help us achieve our shared goal of empowering our patients to reach their highest functional potential.

Position Overview:

We currently have full-time, part-time, and PRN positions available, in the Norcross, Duluth, Suwanee and/or Sugar Hill areas. As a Physical Therapist, you’ll be evaluating and treating a primarily older adult patient population, with various disabilities and injuries. Your role will directly impact the quality of life of our patients, supporting them as they work towards their functional and therapeutic goals.

Key Responsibilities:

Conduct evaluations and provide treatment for patients in the clinic and ALF/ILF setting Develop individualized treatment plans to achieve functional goals Co-sign PTA documentation and complete appropriate documentation for Progress Notes (PN’s), Updated Plan of Care (UPOC), Discharge summaries (D/C’s), etc. Collaborate with patients and their families to promote safety, independence, and mobility

Qualifications:

Valid Georgia Physical Therapist License (required) Experience in treating adult and geriatric populations Ability to work both independently and with team members Passion for providing compassionate care Excellent communication skills and attention to detail

Compensation & Benefits:

Competitive pay rates: $55.00•$70.00 per hour Flexible work schedule: Monday•Friday Full-Time, Part-Time & PRN positions available
