Staff /n Interprets physicianreferrals, as appropriate, and completes musculoskeletal,neuromuscular, cardiopulmonary, and/or integumentary evaluationsutilizing appropriate methods. Establishes a plan of care andimplements individual treatment based on evaluation results andfunctional goals and by utilizing appropriate techniques. Monitorsand evaluates outcomes and appropriately adjusts the individualizedtreatment plan based on the patient’s response to treatment andprovides status updates to physicians and other teammates.

Identifies factors affecting patient’s physical and psychosocialcomfort and attends to these needs. Plans for and completes patientdischarge including providing written home programs, patient/familyteaching, referral for community services, transition planningwithin the continuum of care and ordering equipment asindicated/appropriate. Communicates and collaborates withappropriate stakeholders to assist in achieving optimum care forthe patient. Completes all required documentation followingdepartment policy and accreditation standards with appropriate co-signatures, including patient evaluations, progress notes, teamconference reports, and discharge summaries as appropriate. Recordpatient care and progress of each patient assigned. Completes dailystatistics, billing, and scheduling. Applies insurance guidelinesregarding authorization and number of visits. Responsible formaintaining site/department established productivity standards.

Responsible for understanding and adhering to all billing anddocumentation policies, procedures, and timelines. Responsible forcleaning up after patient treatment, putting away equipment, andkeeping area neat and orderly. Performs various manual techniques,patient positioning, transferring and transporting duties, whichrequire lifting and pushing/pulling, while utilizing propertechnique. Demonstrates skill in use of modalities, exerciseequipment and other devices that are available in area worked perdepartment orientation checklist. Demonstrates knowledge ofindications and contraindications of modalities and exercise.

Promotes patient safety by participating in safety initiatives asrequested and appropriately follows all safety and infectioncontrol measures pertaining to the general working environment.

Participates in performance improvement activities designed toidentify and resolve problems affecting patient care outcomes andto improve quality of care. Manages assigned caseload, informingsite leadership of coverage needs and ability to cover; assistswith coverage, based on department needs and regional needs Pursuesprofessional growth through participation in continuing educationcourses, department in-services, and reading professionalliterature. Orients new staff to clinical and department policiesas requested. Participates in the evaluation process of co-workersby providing verbal or written feedback as requested. May supervisephysical therapy assistants following all applicable state andlocal compliance requirements. Includes delegating patients toassistive personnel while providing direction, recommendationsregarding treatment and ongoing supervision while maintainingresponsibility for patient plan of care and goals. May supervisephysical therapy extenders (rehab aides/techs) in administeringtreatments to patients and will teach them appropriate techniquesto perform with specific patients. May supervise physical therapystudents after completing one year of clinical experience. MinimumJob Requirements Education Doctorate degree in Physical Therapyfrom an accredited school or educational requirements commensuratewith the date of graduation. Maintains continuing educationrequirements as mandated by applicable state professionalstandards.

Certification / Registration

/ License Licensed PhysicalTherapist in the State upon which you work. Basic Life Support(BLS) for Healthcare Providers (obtained within 90 days unlessdepartment leader has determined is not required) by American HeartAssociation (AHA) Work Experience No experience required Knowledge/ Skills / Abilities Intermediate computer skills Excellentcommunication, interpersonal, and organization skills Demonstratedorganization, decision-making, and problem-solving skillsSuccessfully completes all competency assessments/requirementspertaining to provision of patient care and consistent with sitestandards Team members will maintain annual educationalrequirements for hospital-based certifications (e.g. DNV, CARF orothers). Must be able to demonstrate knowledge and skills necessaryto provide care appropriate to the age of the patients served. Mustdemonstrate knowledge of the principles of growth and developmentover the life span and possess the ability to assess datareflective of the patient’s status and interpret the appropriateinformation needed to identify each patient’s requirements relativeto his/her age-specific needs. Physical Requirements and WorkingConditions Must be able to sit, stand, walk, lift, and squatthroughout the workday Must be able to: lift up to 50 lbs. fromfloor to waist. lift up to 10 lbs. from waist to overhead. lift andcarry up to 40 lbs. at waist height a reasonable distance Must beable to: push/pull with 30 lbs. of force. perform a slidingtransfer of 150 lbs. with a second person present Position requiresrepetitive use of hands: Simple grasping
