Job Description:

Physical Therapist – Home Health Join our team as a dedicatedPhysical Therapist in Home Health, where you will help patientsimprove movement, manage pain, and regain independence in thecomfort of their own homes.

Key Responsibilities:

Review patientmedical histories and conduct assessments to diagnose movementdysfunction and identify patient goals. Develop and implementindividualized treatment plans that may include hands-on therapy,exercises, and use of assistive equipment. Provide skilled physicaltherapy services in accordance with a physician’s plan of care,adapting interventions to the home environment. Educate patientsand family members about the recovery process, safe mobility, andhome exercise programs. Monitor and document patient progress,modifying treatment plans as needed to achieve optimal outcomes.

Communicate and collaborate with physicians, nurses, and otherhealthcare professionals to coordinate care. Supervise and instructPhysical Therapist Assistants or Aides as required. Ensure alldocumentation is completed promptly and accurately in compliancewith agency and regulatory standards.

Work Environment:

The role isbased in patients’ homes, requiring travel within the community andadaptability to various home settings. This position requiresstrong communication, organizational skills, and the ability towork independently.
