40 hours – Day/Evening shifts Monday through Friday flexibleshifts. Min requirement of 2 evenings/week.

43 VillageSquare – Chelmsford Job Overview This position evaluates, developsand implements specific treatment programs for individual patientsaccording to the principles and practices of physical therapy. Thetherapist will be educated in evaluating and treating all agegroups to help patients achieve a maximum level of function usingvarious therapeutic exercises, activities, modalities and skills.
The therapist is responsible for documenting all age-appropriatepatient care treatments in the medical record to comply with thepolicy and procedures of the department, organization and JCAHOrequirements. The staff member will act as a liaison with otherdisciplines for problem-solving issues that arise related toclinical therapy services. May provide physical therapy coverage ona rotating bases for weekends and holidays dependent on operationalneed. Minimum Qualifications 1. Bachelors Degree. 2. Graduationfrom an approved school of Physical Therapy. 3. Physical Therapy(PT) License. Licensure in NH may be required depending on assignedterritory and operational need. 4. One (1) year of clinicalexperience. Duties and Responsibilities The duties andresponsibilities listed below are intended to describe the generalnature of work and are not intended to be an all-inclusive list.
Other duties and responsibilities may be assigned. 1. Assesses,re-assesses & treats a variety of diagnoses with a high degree ofprofessional competency & efficiency to ensure quality patientcare; Able to evaluate & treat pediatric, adolescent, geriatric &the general population. 2. Assesses & re-assesses patient pain anddevelops appropriate treatment plan to assist patient in attainingmaximum functional outcomes. 3. Interacts professionally withpatient/family and involves patient/family in the formation of planof care; formulates a teaching plan based upon identified learning,cultural and psychosocial needs. 4. Sets realistic short and longterm goals and plan of care related to patient’s physical therapyneeds within the musculoskeletal, neuromuscular, cardiopulmonary,and integumentary systems. 5. Maintains and completes proper andtimely documentation in the medical record for all patients treatedto promote communication to physicians and to ensure properreimbursement. 6. Contributes to the department to ensure that theoperations are efficient and cost effective; adheres to billingguidelines, charges, insurance limitation & all other departmentaloperations. 7. Prepares department for JCAHO surveys andparticipates in CQI activities. Performs other related duties asassigned and/or necessary to assure appropriate patient care. 8.Attends intra-departmental rounds, team meetings, and caseconferences and staff meetings. Attends professional meetings,courses and conferences to continue education. 9. Followsestablished organizational precautions and procedures in theperformance of all job duties to ensure a safe work environment forself and others. 10. Works directly and collaboratively with theinterdisciplinary health care team, the patient, and the patient’sfamily to promote maximum level of patient safety and independencein discharge planning.
