- Developing and implementing treatment plans thatfocus on the physical, cognitive and emotional needs of patients
- Assessing home environment to ensure safety and providerecommendations for any necessary modifications or equipment needs
- Providing patient and caregiver education

- Candidates must haveMA Physical Therapist (PT) license.
- Minimum of 1-year experienceas a Physical Therapist (PT).
- Ability to work independently andas part of a team while working with a variety of patients andmedical professionals
- Excellent communication and interpersonalskills
- Must possess strong organizational skills and attention todetail
