• Developing and implementing treatment plans thatfocus on the physical, cognitive and emotional needs of patients
• Assessing home environment to ensure safety and providerecommendations for any necessary modifications or equipment needs
• Providing patient and caregiver education

• Excellent communication and interpersonalskills
