Physical Therapist Job Description Plans and conductssegments of treatment programs designed to restore and improvephysical, social, and mental functions while meeting departmentobjectives. Serves as co-leader in an interdisciplinary approach topatient groups. Completes evaluations to ensure quality standardsand addresses assessment areas that require attention. Usesappropriate clinical techniques and tools and establishesindividualized treatment plans for patients. Provides therapyinterventions using appropriate treatment protocols and techniques.

Establishes individualized care and discharge plans and ensuresplans are communicated to appropriate parties. Completescomprehensive and accurate clinical documentation withinestablished deadlines. Supervises assigned assistants. Performsother job-related duties as assigned. Collaborates with alldisciplines to evaluate team goals. Establishes and administers atreatment program with realistic and achievable goals.

Qualifications 1 years of recent experience Graduate of anaccredited school BLS certification Active state licenseDemonstrate the highest level of professionalism when interactingwith patients, families, physicians, and hospital staff members.
