A Brief Overview Responsible for the daily coordination and delivery of physicaltherapy services implementing specific treatment program forindividual patients and patient groups according to the principlesand practices of physical therapy and established objectives of thedepartment. What you will do Evaluates assigned patients, gathering data from a variety ofsources such as patient records, interviews, observation and teammembers. Develops and implements individualized treatment programs forthe patients needing services. Reviews plan of care with patient.

Instructs patient, family, caregivers in home program or exerciseas appropriate. Uses selected activities and/or tasks to promote and maintainhealth, and to achieve treatment goals, establish and modifyintervention and strategies for patients’ treatment program basedon ongoing evaluation. Provides direct therapy services to patients using variousfunctional settings and strategies, plans therapy within thepatient’s individual physical, emotional and intellectualcapabilities as indicated and coordinated with the rehabilitation,medical, and nursing care plans. Prepares documentation as required by the profession and thedepartment, such as evaluation results, individualized treatmentplans, progress notes, and other such reports. Communicates effectively with the patient, family, physician,and other disciplinary team members regarding the patient’s status,progress and needs. Attends care conferences when applicable. Establishes rapport and work cooperatively withinterdisciplinary team members (including family/physician)throughout the patient’s intervention program. Enhances professional growth and development throughparticipation in educational programs, current literature,in-service meetings, workshops, etc. Oversees physical therapy assistants, techs, and students in anefficient and effective manner that ensures safe patient care andsmooth departmental operation. Maintains knowledge of proper use of equipment for patientcare. Essential Qualifications The individual must be able to work the hours specified. To performthis job successfully, an individual must be able to perform eachessential job function satisfactorily including having visualacuity adequate to perform position duties and the ability tocommunicate effectively with others, hear, understand anddistinguish speech and other sounds.
