• Current state license as a Registered Physical Therapist / PT orproactively in the process of the application process for currentstate licensure as a Registered Physical Therapist / PT.

• CEUreimbursement Duties Responsibilities The Physical Therapist / PTis responsible for the assessment of referred patients that requirerehabilitation services which includes; providing direct patientcare to assess their medical condition, functional capabilities,limitations and restrictions and potential for rehabilitation.

Also, the Physical Therapist / PT will establish and administer atreatment program with specific goals determined according to thepatient’s capacity and tolerance under the direction of thePhysician. As well, the Physical Therapist / PT must set realisticand achievable goals for their patients, document and record thepatient’s condition, and educate patients and families in anappropriate physical therapy method.

• Develops a plan of care foreach physical therapy patient.
• Provides skilled physical therapyservices / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care tomeet goals of physical therapy interventions.
• Collaborates withall disciplines to plan and evaluate team goals for each patient.
