Utilizes clinical reasoning to developappropriate treatment plan and goals focused on progressingpatients towards discharge within an established length ofstay./nCollaborates with interdisciplinary team to address allbarriers to discharge. This includes timely equipment trialing andordering, completion of all patient and family training includingissuing of a home exercise program, and planning for continuingservices. Demonstrates flexibility in all aspects of caseloadmanagement including accepting coverage, ensuring that the plan ofcare is communicated, implemented and progressed throughout thelength of stay. Effectively and efficiently manages individual,concurrent, and group treatment sessions based on patient careneeds./nCompletes medical record documentation in a clear, concise,accurate and timely manner, inclusive of daily documentation, goalwriting, functional quality indicator assessment, letters ofmedical necessity, discharge documentation, and charge entry usingapproved formats in compliance with state, federal and intermediaryguidelines and in accordance with the Hospital’s policies andprocedures. Ensures care is provided under signed, valid physicianorders and within insurance approval, as indicated and necessaryfor clinical care of the patient./nParticipates in student programas assigned and appropriate, providing a positive learningenvironment for students with appropriate supervision, developmentof learning objectives and completion of studentevaluations./nActively creates an individualized professionaldevelopment plan with clinical management. Identifies and initiatesown professional growth, actively pursuing professional developmentthrough continuing education, self-study and involvement inprofessional activities and organizations. Applies learnedinformation in order to meet the strategic objectives of theorganization./nActively participates in patient and professionaladvocacy efforts that may include lobbying activities, healthfairs, community level events, professional memberships,fundraising/charitable activities./nAdheres to discipline specificState Practice Act and other regulatory agencies.
