Join our compassionate team in Holmes, NewYork, where you’ll help residents regain mobility, strength, andindependence in a supportive Skilled Nursing Facility (SNF)environment.

Key Responsibilities:

• Perform comprehensiveevaluations to assess residents’ mobility, strength, and therapyneeds.
• Develop and implement personalized treatment plans toimprove residents’ functional abilities and quality of life.
• Provide therapeutic interventions, including exercises andmodalities, to help residents achieve their goals.
• Monitorprogress, adjust treatment plans as needed, and provide educationto residents and their families.
• Collaborate with aninterdisciplinary team to deliver exceptional patient-centeredcare.
• Document assessments, treatments, and progress accuratelyand in compliance with facility and regulatory guidelines.

Qualifications:

• Current Physical Therapist (PT) license in NewYork.
• Graduate of an accredited Physical Therapy program.
• Experience in a Skilled Nursing Facility (preferred but notrequired).
• Strong communication and interpersonal skills.
