Local ContractAs a Physical Therapist in aSkilled Nursing Facility, you will be essential in providinghigh-quality patient care and rehabilitation services. Reporting tothe Rehabilitation Director, you will utilize your core skills inanatomy, physiology, and medical terminology to develop effectivecare plans. Your expertise in discharge planning will ensure smoothtransitions for patients returning home or to communitysettings.

• Conduct thorough evaluations of patients to assess theirphysical condition and functional abilities.
• Develop individualized treatment plans based on patientassessments, goals, and needs.
• Implement therapeutic interventions including manual therapytechniques, exercise programs, and modalities to promote healingand recovery.
• Monitor patient progress and adjust treatment plans asnecessary to ensure optimal outcomes.
• Educate patients and their families on rehabilitationprocesses, home exercise programs, and lifestyle modifications tosupport recovery.
• Collaborate with healthcare professionals to coordinatecomprehensive patient care, including discharge planning andfollow-up services.
• Maintain accurate documentation of patient evaluations,treatment plans, progress notes, and discharge summaries incompliance with regulatory standards.
• Stay current with advancements in physical therapy practicesand participate in ongoing professional development. Qualifications
• Valid Physical Therapist license
• Strong patient care and assessment skills
• In-depth knowledge of anatomy and physiology
• Proficiency in medical terminology and physical therapytechniques
• Experience with discharge planning and care plans
• Familiarity with acute care settings and geriatricspreferred
