Full Time JobDescription:

Provides patient care services along with directtreatment and follow-up. Evaluates patients new to service wherethere are physician orders and develops a plan of care appropriateto the patient’s diagnosis. Documents patient care data accurately,thoroughly, and in a timely matter, reflecting evidence ofimprovement/progress/decline in patient’s condition. Submits thedaily/weekly patient visit schedule to the Scheduler. Communicatesas necessary with intercompany and intracompany contacts. Discussescompany and third party reimbursement policies and guidelines withthe patient/family and discuss treatment plans with the PatientCare Coordinator, physician, and patient/family. Coordinates withPatient Care Coordinator in making referrals to appropriatedisciplines. Provides in-service education, health careprofessional education, and public education in addition tosupervising peers and supporting personnel and students in trainingas needed by the company. Conducts research and assists withorientation of new employees as needed.

Job Requirements:

Minimumbachelor’s degree in physical therapy from an accredited school ofphysical therapy and current licensure from the state where therapywill be provided. Prefer a minimum of one year’s therapeuticexperience in a non-home health care environment. Strong knowledgeof theory and concepts relating to the practice of physical therapyand the ability to evaluate and treat patients for whom aphysician’s order exists.
