Summary:

Responsible for the planning, provision, andcoordination of all aspects of direct patient care in accordancewith established clinic and departmental policies and procedures,maintaining the highest degree of quality care in a safeenvironment. The therapist works collaboratively with other patientteam personnel in maintaining standards of professional physicaltherapy practice.

The staff therapist, using independent judgment,assumes responsibility and accountability for the delivery ofphysical therapy services provided by Physical Therapist Assistantsand Rehab Techs/Aides. May be assigned to orient staff and studentsand assume the duties and responsibilities of the Clinic Directorin the absence of that individual.

Essential Duties andResponsibilities:

Provides high quality patient care and assessmentin accordance with established practice standards andclinic/departmental policies in a safe environment.

Performs patientevaluation, summarizes objective clinical findings, and develops anaccurate individualized plan of care. Ensures that goals arefeasible, based upon findings and patient expectations.

Providestreatment in accordance with physicians orders and established planof care.

Evaluates effectiveness of treatment plan and revises planof care as indicated.

Communicates/collaborates with patient,family, caregivers, significant other and other members of thehealthcare team to promote maximum benefit ofcare.

Communicates/collaborates with the patients referral sourcesuch as the physician, NCM, ADJ, etc. regarding the patients careand progress to promote maximum benefit of care.

Assess educationalneeds of the patient, family, caregiver, or significant other andother members of healthcare team to promote maximum benefit ofcare.

Demonstrates appropriate knowledge and correct application ofclinical techniques, procedures and programs for clinical settingand patient/client populations served.

Plans and makes appropriatedischarge plans.

Assesses the risks for safety and implementsappropriate precautions.

Complies with appropriate and approvedsecurity and safety standards.

Utilizes appropriate leadershipskills in delegating, organizing, and educating coworkers andstaff.

Participates in and contributes to quality improvementprocess.

Makes effective use of patient schedule and time byachieving various efficiency standards such as skilled units pervisit, visits per day, and evaluations plus two-skilledunits.

Completes timely, competent, and compliant documentationusing AgileRPM.Travels to other local clinics as assigned.

Qualifications:

Masters or Doctorate degree inPhysical Therapy.

Current Physical Therapist license, registrationand/or certification as per state regulations.
