We are comprised of a 25-bedcritical access hospital and offer a range of services in FortBragg. Job Summary:

Plans andconducts segments of treatment programs designed to restore andimprove physical, social and mental functions, while meetingdepartment objectives. Serves as co-leader in interdisciplinaryapproach to patient groups.

Job Requirements:

Education and WorkExperience:

• Bachelors Degree or equivalent (some certificationswill require Bachelors): Required

Master’s Degree:

PreferredLicenses/Certifications:

• Physical Therapy licensure instate of practice: Required
• Cardiopulmonary Resuscitation (CPR) orBasic Life Support (BLS

OR HS-BLS OR RQIBLS

) certification:

Required

• Valid Drivers License (DL) and must be at least 21 yrs ofage or older: Required
• Physical Therapist (PT): Required
• BasicLife Support (BLS) or Basic Life Support (BLS) or RQIBLS orHeartsaver CPR (CPR):

RequiredEssential Functions:

• Completesevaluations to ensure quality standards and addresses assessmentareas that require attention. Uses appropriate clinical techniquesand tools and establishes individualized treatment plans forpatients.
• Provides therapy interventions using appropriatetreatment protocols and techniques. Establishes individualized careand discharge plans and ensures plans are communicated toappropriate parties.
• Completes comprehensive and accurate clinicaldocumentation within established deadlines.
• Supervises assignedassistants.
• Performs other job-related duties asassigned.
