Weare looking for a talented Physical Therapist with Home Healthexperience to work at an excellent healthcare facility in FortBragg, California. This is a fantastic opportunity to build uponyour physical therapy skills while advancing your PT career. Thejob will entail providing therapy and rehabilitation services topatients at the facility. You care about patients and are committedto helping them recover from their physical ailments. You will workwith new and ongoing patients to create individualized plans oftreatment that consist of appropriate exercises, procedures, andtechniques to promote effective and safe recovery.

JobResponsibilities:

Demonstrate clinical reasoning while evaluatingpatients, identifying diagnosis, and developing a comprehensiveplan of care.

Perform age-appropriate competency skills in area ofpractice and with respect to individual patients problems.

Followintervention protocols and alter plan of careappropriately.

Document patient progress according to approvedmedical and facility documentation procedures.

Discharge patientwhen goals have been met or medical necessity no longer warrantsintervention.

Work cooperatively with supervisors and peers tocontribute to the overall productivity of the team.
