Home Health Physical Therapist- $10,000 SIGN ON BONUSCare IVHome Health is a locally owned and operated home health agency inFayetteville AR, seeking a full-time Physical Therapist to join ouresteemed team. Requirements:

ActiveArkansas Physical Therapy license or registered in the state ofArkansas.

A minimum of three professional references on theapplication or upon initial contact.

Home Health experience is aplus but not required.

Duties may include, but are not necessarilylimited to:

Visit patients within our Hot Springs service area toprovide care in their homes or assisted living facilities.

Developand execute personalized physical therapy care plans for eachpatient.

Record patient progress, including admissions, discharges,OASIS assessments, and regular updates to the Director.

Complete anyother assigned tasks.
