Job Summary:

/nPlans and conducts segments of treatment programs designed torestore and improve physical, social and mental functions, whilemeeting department objectives. Serves as co-leader ininterdisciplinary approach to patient groups. /n

• Physical Therapy licensure in state of practice: Required/n
• Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLSOR

HS-BLS OR RQIBLS

• Valid Driver’sLicense (DL) and must be at least 21 yrs of age or older: Required/n
• Physical Therapist (PT): Required /n
• Basic Life Support(BLS) or Basic Life Support (BLS) or RQIBLS or Heartsaver CPR(CPR): Required /n /n/n/n

Essential Functions:

/n /n

• Completesevaluations to ensure quality standards and addresses assessmentareas that require attention. Uses appropriate clinical techniquesand tools and establishes individualized treatment plans forpatients. /n
• Provides therapy interventions using appropriatetreatment protocols and techniques. Establishes individualized careand discharge plans and ensures plans are communicated toappropriate parties. /n
• Completes comprehensive and accurateclinical documentation within established deadlines. /n
• Supervises assigned assistants. /n
• Performs other job-relatedduties as assigned.
