Home Health Physical Therapist:

Provide one-on-one care in clients’ homes, ensuring they receive top-quality therapy. Perform mobility and functional assessments , develop individualized care plans , and help clients regain strength and independence. Educate clients and their families on exercises, safety measures, and strategies to enhance daily living. Collaborate with a multidisciplinary team to deliver holistic, patient-centered care . Document progress and care plans efficiently using state-of-the-art EMR technology .
