The Physical Therapist II evaluates patients’ conditions, developspersonalized treatment plans, and implements therapeuticinterventions to optimize functional abilities. This position isable to effectively and efficiently treat all patients in theirclinical practice setting and complete all physical therapy jobfunctions with minimal guidance. This position initiatescommunication amongst inter-disciplinary team members, treatsfamiliar patients with confidence, and advocates for their patientsand team. This role requires independent practice in the primarysetting and may require occasional guidance needed when covering incomplex or cross-coverage situations.
Responsibilities:
Evaluatesand treats all diagnoses and more complex patients. Conductsthorough evaluations of patients’ physical abilities, medicalhistory, and functional limitations to develop individualizedtreatment plans. Administers therapeutic exercises and otherphysical therapy interventions to improve patients’ strength,flexibility, and range of motion. Educates patients and theirfamilies on treatment plans, home exercises, and self-managementtechniques to improve recovery outcomes. Collaborates with othersin developing and implementing discharge plans and recommendationsfor continued care. Monitors and documents patients’ progress,treatment outcomes, and responses to interventions so thattreatment plans can be adjusted as needed. Collaborates on teamsfrom different departments to ensure coordinated patient care andholistic treatment outcomes. Performs other duties as assigned.

EDUCATION
Required:
Bachelor’s degree in PhysicalTherapy
EXPERIENCE
Required:
3 years
LICENSURE/CERTIFICATION/REGISTRY/LISTING
Required:
North Carolinalicense by the NCPT Board of Physical Therapy Examiners; BLS(CPR)-American Red Cross or AHA Healthcare Provider
