Overview:

The Physical Therapist II evaluates patients?conditions, develops personalized treatment plans, and implementstherapeutic interventions to optimize functional abilities. Thisposition is able to effectively and efficiently treat all patientsin their clinical practice setting and complete all physicaltherapy job functions with minimal guidance. This positioninitiates communication amongst inter-disciplinary team members,treats familiar patients with confidence, and advocates for theirpatients and team. This position may require mentoring/guidance incomplex situations and may have provide cross coverage to othersettings.

Responsibilities:

Evaluates and treats all diagnoses and more complex patients. Conducts thorough evaluations of patients’ physical abilities,medical history, and functional limitations to developindividualized treatment plans. Administers therapeutic exercises and other physical therapyinterventions to improve patients’ strength, flexibility, and rangeof motion. Educates patients and their families on treatment plans, homeexercises, and self-management techniques to improve recoveryoutcomes. Collaborates with others in developing and implementing dischargeplans and recommendations for continued care. Monitors and documents patients’ progress, treatment outcomes, andresponses to interventions so that treatment plans can be adjustedas needed. Collaborates on teams from different departments to ensurecoordinated patient care and holistic treatment outcomes. Performs other duties as assigned.

Qualifications:

EDUCATION

Required:

Bachelor’s degree in Physical Therapy

EXPERIENCE

Required:

3 years

LICENSURE/CERTIFICATION/REGISTRY/LISTING

Required:

North Carolinalicense by the NCPT Board of Physical Therapy Examiners; BLS(CPR)-American Red Cross or AHA Healthcare Provider
