HOME CARE PHYSICAL THERAPIST

Per Diem, PRN Job Description:
Provides patient care services along with direct treatment andfollow-up. Evaluates patients new to service where there are physicianorders and develops a plan of care appropriate to the patient’sdiagnosis. Documents patient care data accurately, thoroughly, and in atimely matter, reflecting evidence of improvement/progress/declinein patient’s condition. Submits the daily/weekly patient visit schedule to theScheduler. Communicates as necessary with intercompany and intracompanycontacts. Discusses company and third party reimbursement policies andguidelines with the patient/family and discuss treatment plans withthe Patient Care Coordinator, physician, andpatient/family. Coordinates with Patient Care Coordinator in making referralsto appropriate disciplines. Provides in-service education, health care professionaleducation, and public education in addition to supervising peersand supporting personnel and students in training as needed by thecompany. Conducts research and assists with orientation of new employeesas needed.
Job Requirements:
Minimum bachelor’s degree in physical therapy from anaccredited school of physical therapy and current licensure fromthe state where therapy will be provided. Prefer a minimum of one year’s therapeutic experience in anon-home health care environment. Strong knowledge of theory and concepts relating to thepractice of physical therapy and the ability to evaluate and treatpatients for whom a physician’s order exists.
