We are seeking a compassionate and skilledPhysical Therapist to join our team in a long term care/rehabilitation centersetting. This role is ideal for a dedicated professional who is passionateabout improving the mobility, independence, and quality of life for elderly andrehabilitative patients.

Key Responsibilities:

+ Conduct comprehensive physical therapy evaluations and develop individualized treatment plans. + Deliver therapeutic interventions including exercises, manual therapy, and mobility training. + Collaborate with an interdisciplinary team including physicians, nurses, and other therapists. + Monitor and document patient progress, adjusting care plans as needed. + Educate patients and caregivers on home exercise programs and safety techniques. + Maintain compliance with all regulatory and documentation standards.

Qualifications:
