13 Weeks We are seeking a compassionate and skilledPhysical Therapist to join our team in a long term care/rehabilitation centersetting. This role is ideal for a dedicated professional who is passionateabout improving the mobility, independence, and quality of life for elderly andrehabilitative patients.

Key Responsibilities:

Conduct comprehensive physical therapy evaluations and develop individualized treatment plans. Deliver therapeutic interventions including exercises, manual therapy, and mobility training. Collaborate with an interdisciplinary team including physicians, nurses, and other therapists. Monitor and document patient progress, adjusting care plans as needed. Educate patients and caregivers on home exercise programs and safety techniques. Maintain compliance with all regulatory and documentation standards.

Qualifications:
