Discovery AtHome is hiring a Registered Physical Therapist for our Keller,Southlake, Alliance area In this role you will provide skilledPhysical Therapy services on an intermittent basis to clients intheir homes in accordance with physician orders.
Responsibilities:
Provides initial and ongoing comprehensive assessments of theclient’s needs, including Outcome and Assessment Information Set(OASIS) assessments. Establishes a treatment plan in consultationwith the physician and revises it with the approval of thephysician as necessary. Assists in development and implementationof the interdisciplinary client care plan as it pertains toPhysical Therapy. Assists the physician in evaluating patients byapplying diagnostic and prognostic muscle, nerve, joint andfunctional ability tests. Treats clients to relieve pain, developor restore functional and maintain maximum performance, usingphysical means, such as exercise, massage, heat, water, light andelectricity. Consults and familiarizes staff regarding theestablished plan. Teaches, supervises, and counsels the family andclient in the physical therapy program. Instructs client in careand use of wheelchairs, braces, crutches, canes and prosthetic andorthotic devices. Provides the equipment required to carry out theplan. Arranges for out-patient service for procedures which cannotbe given at home. Records on a timely basis all evaluation data,treatments, and client’s response to therapy intervention. Alsorecords quality assurance data. Prepares an assessment of theclient’s condition and periodic evaluations and summaries.
Supervises Physical Therapy Assistants and paraprofessionals atleast once monthly. Records and reports to the physician theclient’s reaction to treatment and /or any changes in the client’scondition. Communicates with the physician at least every 30 daysor whenever changes occur regarding the client’s status and obtainsadditional orders, if necessary. Communicates with other teammembers (including Home Health Aides) and, when appropriate,instructs them in certain phases of physical therapy that they mayuse while working with the client. Writes clinical/progress noteson the date of the visit and incorporates same in the client’srecord within 72 hours. Participates in case conferences, physicaltherapy staff meetings, agency activities and in-service programsas necessary. Supervises the home health aide every two weeks whenphysical therapy is the only professional discipline involved inthe client’s care and supervises the Physical Therapy Assistant atleast monthly. Participates with staff, clients, and physicians indischarge planning activities and completes a Physical TherapyDischarge Summary. Understands and adheres to established policiesand procedures of agency. Participates in Quality Assessment andPerformance Improvement activities as assigned. Attends all patientcare conferences as scheduled. Completes and submits OASISassessments, reassessments, transfers, resumptions of care,discharges, visit notes and significant change in condition inaccordance with agency defined time frames. Obtains a medicalhistory, particularly as it relates to the present condition.
Evaluates the client, the client’s significant other(s) and thehome situation to determine what instruction will be required, whatassistance will be available from the family in caring for theclient, and what other Agency/community services will be required.
Assures that the physician’s orders are appropriate and discussesnecessary changes.
Qualifications:
Must be a graduate of a PhysicalTherapy school approved by the Council of Medical Education of theA.M.A. in collaboration with the American Physical TherapyAssociation. Must be licensed or registered by the State. Must haveat least one year of institutional experience. Must have at least 2years of home care experience and at least one year supervisoryexperience. (2years preferred)
