Job Description Job Description BAYADA Home Health Care iscurrently seeking an experienced Physical Therapist, PT, for a fulltime opportunity performing home health visits for our office,located in Concord NH and surrounding areas. This office servicesadult and geriatric clients on a per visit basis in territoriesthroughout the Sunapee Lakes region in New Hampshire. One yearprior clinical experience as a licensed Physical Therapist isrequired. As a home care Physical Therapist, you will be anintegral member of a multi-disciplinary health care team thatprovides rehabilitative care and skilled nursing to clients,affording them the opportunity to receive the medical care requiredto remain at home. • Physical Therapist include:
• Make home visits to clients indesignated geographic territories.
• Perform diagnostic tests andmeasurements, such as the mobility/range of joints, transferstatus, stability, patterns and appearance of ambulation, strengthand endurance of muscles, balance testing, and safety assessments
• Develop and implement appropriate individualized care plans,including manual therapeutic exercises, gait training, balance, andother interventions.
• Continually assess and revise the PhysicalTherapy
• PT care plan, and participate with nursing in themultidisciplinary care plan, as appropriate.
• Educate and instructclients, family members, or other client representatives, inrehabilitative care and activities necessary to promote theclient’s health, safety, and independent living.
• Accuratelydocument observations, interventions, and evaluations pertaining toclient care management and services provided, utilizing astate-of-the-art touch pad tablet. Qualifications for a PT
• Physical Therapist include:
• A current New Hampshire PhysicalTherapist
• PT license.
• A minimum of one year of recent workexperience as a Physical Therapist
• PT.
• Graduation from aprogram approved by the American Physical Therapy Association orthe Committee on Allied Health Education and Accreditation of theAmerican Medical Association, as indicated by school transcript ordiploma.
• Ability to work independently and manage timeeffectively.
• Strong interpersonal, organizational, and problemsolving skills.
