Education:
Bachelor of Science Degree in physicaltherapy from an APTA approved program.
Experience:
One year ofacute care experience. Additional experience in home health carepreferred.
Certificate/License:
Possession of a current license topractice as a physical therapist in the State of Connecticut.
General Summary of Duties:
The provision of physical therapy topatients in their homes under written orders from qualifiedphysicians according to established Agency policies and procedures.
Typical Physical Demands:
Requires full range of body motion.
Requires ability to perform heavy work. Requires balance skills.
Requires ability to ascend/descend stairs. Requires ability toperform floor-to-stand transfers. Requires ability to kneel and sitfor extended periods. Requires ability to lift, transfer, andperform gait training on all surfaces with patients. Requiresability to lift and carry items weighing up to 50 pounds. Requirescorrected vision and hearing to normal range. Requires workingunder stressful conditions. Requires limited exposure tocommunicable diseases.
Typical Work Conditions:
Work is performedin patients’ homes and in an office environment.
Knowledge, Skillsand Requirements:
Knowledge of current APTA guidelines and standards. Knowledgeof Agency policy, regulations, and procedures to administer patientcare. Knowledge of current modalities and medical equipment relevantto patient care. Skill in identifying patient problems andrecommending solutions. Skill in documentation in clinical records. Skill inestablishing and maintaining effective working relationships withpatients/families, physicians, co-workers, and the community.
Ability to communicate clearly. Possesses adequate transportation to carry out jobrequirements. Must show proof of satisfactory automobile insurancewith minimum coverage to be determined by Agency policy when usingautomobile for transportation. Possession of an individualmalpractice insurance policy. Willingness to provide service in anypart of agency geographic area, according to agency need.
