Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. In this role, you will be responsible for evaluating and treating patients with various physical ailments, helping them regain mobility and improve their quality of life. You will work in a collaborative environment, providing patient-centered care in settings such as outpatient clinics, hospitals, and home community care.
Responsibilities
Conduct comprehensive assessments of patients to determine their physical therapy needs.
Develop individualized treatment plans based on patient evaluations and goals.
Implement therapeutic interventions to improve patients’ strength, flexibility, and mobility.
Educate patients and their families about treatment plans, exercises, and self-care strategies.
Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.
Collaborate with other healthcare professionals to ensure coordinated patient care.
Document all patient interactions and treatment outcomes in accordance with medical standards.
Participate in discharge planning to ensure smooth transitions for patients leaving care.
Requirements
Valid state license to practice as a Physical Therapist.
Strong knowledge of anatomy and physiology relevant to physical therapy practices.
Familiarity with medical terminology and documentation systems.
Experience in outpatient settings preferred; acute care experience is a plus.
Ability to develop effective discharge plans for patients transitioning from acute care or rehabilitation settings.
Excellent communication skills for interacting with patients, families, and healthcare teams.
