Orthopedic At-Home Physical Therapist

  • Per Diem JAG PhysicalTherapy is seeking to hire a skilled Physical Therapistspecializing in orthopedic rehabilitation in the home setting forpatients recovering from total hip and knee replacement surgeriesand other orthopedic related conditions.

We are looking for a compassionateand motivated physical therapist to join our team and make adifference in the lives of those recovering and returning to theirprior level of function. Our physical therapists will be requiredto evaluate and treat the patients for the duration of their planof care in their home setting with the goal of transitioning intoone of our JAG outpatient orthopedic locations for continuity ofcare. What You’llNeed:
  • Bachelor’s or Master’s Degree required, Doctorate degreepreferred
  • Licensed Physical Therapist in the state practicing in
  • Completed state regulated CEU requirement in appropriate state(NY, NJ)
  • Valid CPR License
  • Experience/knowledge with an EMRsoftware is preferred
  • Excellent oral, written communication
  • Strong clinical decision-making skills
  • Excellent work ethic anddedication to patient success
  • Basic computer skills
  • Works as ateam member and individually with minimal supervision
  • Maintains apositive, professional demeanor at all times
What You’ll Do:
  • Educate patient on safe bed mobility, transfers and ambulation inconsideration of post operative precautions
  • Transfer, ambulateand perform stair negotiation with patients to determine they aresafe for discharge to home
  • Collects and records patient care datathat follows format and guidelines of the patient care service,regulatory agencies and third-party payers which reflects anunderstanding of reimbursement systems and their impact on patientcare.
  • This includes, but is not limited to, initial evaluation,daily notes, reassessments/progress reports, discharge plans andcommunication with members of the team.
  • Additionally, completesdaily billing and follows department guidelines for productivity.
  • Prepares for the evaluation in accordance with departmentpolicies/procedures and standards of practice including obtainingpatient history, exhibiting knowledge of patient diagnoses andresearching unfamiliar diagnoses.
  • Selects and administers testsand measures.
  • Additionally, synthesizes evaluation results toformulate the diagnosis, goals, prognosis and plan of care.
  • Establishes treatment plan based on evaluation results, goals andbest practice. Collaborates with interdisciplinary team, patientsand patient’s support system. Assesses and re-assesses need formodifications to plan of care and goals.
  • Provideseducation/training to patient and/or patient support system
  • Functional Training-This includes but is not limited to ADLs/IADLs,therapeutic activities (e.g. transfers, bed mobility),rehabilitation equipment prescription and training and communityreintegration
  • Responsible for treatment, plan of care, exerciseprescription for patient caseload
  • Provides interventions topatients that are safe, effective, and in compliance with JAG
  • Provide data, note writing using Prompt EMR for all treatmentsprovided
  • Attends annual JAG Billing and Coding seminar
  • Communicate with Manager/Patient Access team for schedulingevaluations once demographics are received
