The Physical Therapist will primarily provideoutpatient rehabilitation services in an in-home setting for theolder adult population. The in-home setting could be a patient’sprivate residence or in a patient’s room in an independent livingfacility. Powerback Rehabilitation To You allows the PhysicalTherapist to evaluate and treat in the most functional environmentwhich is the patient’s home. The Physical Therapist assesses theneed for, develops, and delivers physical therapy programs tofacilitate rehabilitation. The Physical Therapist identifies theneed for and makes referral(s) to other disciplines to address thecomprehensive needs of patients. They implement individualizedtherapy programs designed to restore, reinforce, and enhanceperformance. In addition, the Physical Therapist instructs,educates and trains patients and caregivers in those skills andfunctions essential for promoting independence and productivity, inorder to diminish or correct pathology. They direct activities topromote and maintain health. This position provides a flexibleschedule and work environment in various settings. 1. As necessaryor applicable, supervision of Physical Therapy Assistants,temporary licensees, students and support personnel in accordancewith state licensure requirements and professional standards.2.Responsible for adherence to department and patient treatmentschedule, timely clinical and billing documentation requirements aswell as work related meetings.3. Attends and contributes to patientcare, staffing conferences and other related meetings.4. Makesreferrals to other disciplines and agencies in order to address thecomprehensive needs of the . Consults with other members of therehabilitation team to select the most appropriate therapeuticregimen consistent with the needs and capabilities of each . Adaptsprogram and/or plan of care according to the needs of theindividual . Promotes a safe environment and enforces theelimination of fire and safety hazards.8. Orders supplies andequipment as necessary.9. Cleans, maintains, identifies, andreports the need for repair of equipment as necessary.10.
Putspatient service first:
ensures that patients and families receivethe highest quality of service in a caring and compassionateatmosphere which recognizes the individuals’ needs andrights.11.Screens, examines and evaluates patients, includinghistory, systems review, and application of appropriate tests andmeasures, synthesis information and interprets examination findingsin order to establish a diagnosis, identify impairments, determinethe predicted level of improvement and the time required to achieveit, identify precautions/contraindications and design and plan ofcare. Develops appropriate treatment goals and methods incollaboration with the patient and caregivers, implements thephysical therapy treatment plan, and completes all relateddocumentation and record keeping regarding these services. Servicesinclude but are not limited to application of therapeutictechniques in areas such as:
- Therapeutic exercise includingaerobic capacity conditioning, balance coordination, agilitytraining, body mechanics and postural stabilization, flexibilityexercises, gait and locomotion training, neuromotor developmenttraining, relaxation, strength, power, and endurance training
- Functional training
- Manual therapy
- Airway clearance techniques
- Integumentary repair and protection
- Use of electrotherapeutic,physical agent and mechanical modalities
- Health and Wellness
- Education on Chronic Disease Management
- Falls Assessment andInterventions to reduce fall risk
- Case Management in the home andcommunity12.
Designs, fabricates, fits and/or applies adaptivedevices, and trains patients in the use of adaptive devicesincluded orthotics and prosthetics.13. Selects appropriateactivities suited to the individual’s physical and mental capacity,intelligence level and interest.14. Instructs patient in the skillsand techniques required for participation in mobility activitiesand evaluates patient progress, attitudes, and behavior as relatedto rehabilitative potential.15. Performs routine reexamination asneeded/required to modify/progress plan of . Incorporates healthand wellness programming into treatment plans for . Performs homeand community mobility assessments and provides recommendations forsafe mobility in the home and community space.18. Performs otherrelated duties as required.
Qualifications:
1. Must have a validPhysical Therapy license in the state(s) of practice, or proof oflicense eligibility; and2. They must have a Master’s degree inPhysical Therapy; or3. They must have a Master’s degree in aPhysical Therapy related field in addition to a Bachelor’s degreein Physical Therapy; or4. They must have a Bachelor’s degree inPhysical Therapy plus 5 years of progressively responsible PTexperience.5. The qualifying Physical Therapy degree earned outsideof the U.S. must be evaluated to be the U.S. equivalent to a degreein Physical Therapy.6. Must have a valid driver’s license orreliable, independent transportation to and from the patients’residence
