We are currently seeking a dedicated, licensed PhysicalTherapist to join our team and provide expert rehabilitationservices to our residents.

Job Description:

As a PhysicalTherapist, you will work with a diverse population of patients,creating and implementing individualized treatment plans thatenhance mobility, reduce pain, and improve the overall quality oflife for our residents. You will collaborate closely with a team ofhealthcare professionals to provide comprehensive care, promotingindependence and functional improvement.

Key Responsibilities:

Assess, develop, and implement individualized physical therapytreatment plans for residents based on their medical conditions andphysical limitations. Conduct evaluations and re-evaluations ofpatients to track progress and adjust treatment plans as necessary.

Provide hands-on therapy, including therapeutic exercises, manualtherapy, and mobility training to help patients improve strength,balance, and function. Document all patient interactions andtreatments in the medical record in compliance with facilitypolicies and regulatory requirements. Educate patients and theirfamilies on exercises and techniques to improve mobility and managephysical impairments. Collaborate with a multi-disciplinaryhealthcare team, including physicians, nurses, and occupationaltherapists, to coordinate care plans. Ensure a safe and supportiveenvironment by monitoring patient responses to treatment andensuring the correct use of equipment and adaptive devices.

Maintain accurate and timely documentation in the patient’s medicalrecords.

Qualifications:

Graduate of an accredited Physical Therapyprogram. Current, valid Physical Therapy license in the state ofNew York. Previous experience in long-term care or geriatrics ispreferred, but not required. Strong communication and interpersonalskills with the ability to work collaboratively within a team.

Compassionate, patient-focused attitude with a dedication toimproving the lives of others. Ability to work effectively withelderly patients and those with diverse medical conditions.

Excellent organizational skills and attention to detail. Ability towork in a fast-paced, dynamic environment.
