• Evaluate and assess patient todetermine their appropriateness for care
• Utilize the availabletools and resources to develop patient-specific care plans with theappropriate level and frequency of care
• Communicate with the careteam, physician, payors, patients, and families to address careneeds and fulfill the patient’s care plan
• Deliver high-qualityskilled care and supervise the care of other care team members
• Thoroughly document care delivery daily in our EMR system
