Description Summary:

The Physical Therapist has theresponsibility and accountability for assessing, planning,evaluating and implementing care for the patients assigned physicaltherapy. The Physical Therapist is responsible for adhering to allstandards of Physical Therapy Practice Act as they apply toproviding technical therapy care, supplies, equipment andinterventions to a designated patient population. Responsibilities

• Gathers pertinent data from chart and other care givers;discusses with physician as necessary
• Independently assesses allpatient types as assigned
• Demonstrates ability to recognizeadditional needs during evaluation; exercises good judgment indecision making when requesting, scheduling and/or performingadditional testing
• Interprets results and formulates relevant,measurable, realistic and attainable goals
• Reassesses patientprogress regularly per department and regulatory policy, or whenneed arises
• Identifies appropriate equipment, discusses withpatient and family and makes

Requirements:

Education/Skills

• Bachelor’s, Master’s, or Doctorate of Physical Therapy degree froman accredited college or university is required. Licenses,Registrations, or Certifications
• LPT License in state ofemployment
