Job Summary:

We are seeking a qualified and licensed Physical Therapist to assess, plan, and implement rehabilitative programs that improve mobility, relieve pain, and restore physical function in patients. The ideal candidate will be adept at evaluating patient conditions and delivering evidence-based interventions across clinical and/or home care settings.

Key Responsibilities:

Perform comprehensive physical therapy assessments to determine patients functional capabilities, treatment needs, and personalized rehabilitation goals.
Participate in interdisciplinary team meetings to contribute to the development and review of individualized care plans.
Administer prescribed physical therapy treatments in accordance with clinical guidelines, care plans, and physician directives.
Provide therapeutic interventions including, but not limited to, active/passive exercise, neuromuscular re-education, gait training, functional ADL training, and use of modalities such as ultrasound, electrical stimulation, heat, and hydrotherapy.
Supervise and mentor Physical Therapy Assistants in executing Functional Maintenance Programs (FMP), ensuring adherence to treatment protocols and quality standards.
Educate patients, families, and caregivers on therapeutic techniques, mobility aids, and the proper use and maintenance of assistive equipment.
Accurately document assessments, treatment plans, interventions, progress notes, and discharge summaries in compliance with regulatory and organizational standards.

Qualifications:

Bachelor s degree in Physical Therapy with a minimum of 5 years of relevant clinical experience.
Advanced degree (Master s or Doctorate in Physical Therapy) is preferred.
Active or pending state licensure as a Physical Therapist (new graduates welcome to apply).
Minimum of 2 years experience working in a clinical environment, preferably with geriatric or medically complex populations.
