Job Description Mystic Healthcare, a 100 bed SNF located inMystic, CT., is currently seeking a Physical Therapist to join ourteam of healthcare heroes. Responsibilities
- Review request forphysical therapy and physicians’ orders
- Completes screening
- Interviews residents to determine treatment type and scheduletherapy
- Participate in the development of resicent assessments(MDS) and care plans
- Brief resident of procedures involved in ptand prepare necessary equipment
- Assist in transporting residentsto and from therapy
- Determine proper equipment usage,applicantion and body position, and make adjustments as necessaryin the administration of treatments
- Ensure that all therapistnotes are informative and descriptive of the care provided
- Consult with physician/staff as necessary Must have an unencumberedPhysical Therapy License
