The payrange for this position is listed below. ABrief Overview Responsible for the daily coordination and deliveryof physical therapy services implementing specific treatmentprogram for individual patients and patient groups according to theprinciples and practices of physical therapy and establishedobjectives of the department. What you will do Evaluates assignedpatients, gathering data from a variety of sources such as patientrecords, interviews, observation and team members. Develops andimplements individualized treatment programs for the patientsneeding services. Reviews plan of care with patient. Instructspatient, family, caregivers in home program or exercise asappropriate. Uses selected activities and/or tasks to promote andmaintain health, and to achieve treatment goals, establish andmodify intervention and strategies for patients’ treatment programbased on ongoing evaluation. Provides direct therapy services topatients using various functional settings and strategies, planstherapy within the patient’s individual physical, emotional andintellectual capabilities as indicated and coordinated with therehabilitation, medical, and nursing care plans. Preparesdocumentation as required by the profession and the department,such as evaluation results, individualized treatment plans,progress notes, and other such reports. Communicates effectivelywith the patient, family, physician, and other disciplinary teammembers regarding the patient’s status, progress and needs. Attendscare conferences when applicable. Establishes rapport and workcooperatively with interdisciplinary team members (includingfamily/physician) throughout the patient’s intervention program.

Enhances professional growth and development through participationin educational programs, current literature, in-service meetings,workshops, etc. Oversees physical therapy assistants, techs, andstudents in an efficient and effective manner that ensures safepatient care and smooth departmental operation. Maintains knowledgeof proper use of equipment for patient care. EssentialQualifications The individual must be able to work the hoursspecified. To perform this job successfully, an individual must beable to perform each essential job function satisfactorilyincluding having visual acuity adequate to perform position dutiesand the ability to communicate effectively with others, hear,understand and distinguish speech and other sounds.
