Our team is committedto delivering compassionate care in a supportive and collaborativeenvironment. We are seeking a PRN Physical Therapist (PT) to joinour dedicated rehabilitation team. This position offers flexibilityand the opportunity to make a meaningful impact on residents’ livesin a rewarding setting.

Position Summary:

As a PRN PhysicalTherapist, you will evaluate and treat residents to improve theirmobility, strength, and functional independence. You will workcollaboratively with a multidisciplinary team to deliverhigh-quality, individualized care.

Key Responsibilities:

Performcomprehensive evaluations of residents’ physical abilities andneeds. Develop and implement personalized treatment plans toimprove strength, mobility, balance, and overall function. Providetherapeutic interventions, including exercises, manual therapy, andtraining on assistive devices. Monitor residents’ progress andmodify therapy plans as needed to achieve goals. Collaborate withan interdisciplinary team to ensure holistic care. Educateresidents and their families on therapy goals, exercises, andstrategies for continued improvement. Accurately documentevaluations, treatment sessions, progress notes, and dischargeplans in compliance with regulatory and facility standards.

Participate in quality improvement initiatives and contribute tothe rehabilitation department’s success.

Qualifications:

Doctorateor Bachelor’s/Master’s degree in Physical Therapy from anaccredited program. Current and valid Physical Therapist license inthe state of Colorado. Experience in skilled nursing orrehabilitation settings is preferred. Strong communication andteamwork skills with a resident-centered approach. Ability to workindependently and adapt to a flexible PRN schedule. Dedication toproviding compassionate and effective therapy services.

What WeOffer:

Hourly rate range: $50.00 – $55.00 per hour based onqualifications. Flexible PRN scheduling to fit your availability.
