Pay rate annually:

• 89K-90K with bonus incentives, depending on experience.
• _ Work Schedule Mon – Fri (40hrs/week) _
• ••No Weekends ••_ •Duties include:•Patient assessment, develop care plans to treat adult/geriatric patients • Work with a multidisciplinary team and make appropriate decisions based on each patient’s functional needs.

• Work with senior care patients, and some patients with dementia
• Charting/data entry of patient records
• Flexible case loads

• Requirements Include:

• Graduate of an accredited Physical Therapy program
• Valid PA state Physical Therapist license (or eligible for a PA state license)
• Prior experience working in an outpatient or memory care setting _
