BAYADA Home Health Care iscurrently seeking an experienced Physical Therapist, PT, for a fulltime opportunity performing home health visits for our seniorliving office, located in Blue Bell, PA. This office services adultand geriatric clients on a per visit basis in territoriesthroughout the Mainline area. As a home care Physical Therapist,you will be an integral member of a multi-disciplinary health careteam that provides rehabilitative care and skilled nursing toclients, affording them the opportunity to receive the medical carerequired to remain at home. • Physical Therapist include:
• Make home visits to clients indesignated geographic territories.
• Perform diagnostic tests andmeasurements, such as the mobility/range of joints, transferstatus, stability, patterns and appearance of ambulation, strengthand endurance of muscles, balance testing, and safety assessments
• Implement integumentary protective interventions to prevent woundformation and promote wound repair utilizing evidence-baseddressings and topical agents.
• Develop and implement appropriateindividualized care plans, including manual therapeutic exercises,gait training, balance, and other interventions.
• Continuallyassess and revise the Physical Therapy
• PT care plan, andparticipate with nursing in the multidisciplinary care plan, asappropriate.
• Educate and instruct clients, family members, orother client representatives, in rehabilitative care and activitiesnecessary to promote the client’s health, safety, and independentliving.
• Accurately document observations, interventions, andevaluations pertaining to client care management and servicesprovided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT

• Physical Therapist include:
• A currentPA Physical Therapist
• PT license.
• Graduation from a programapproved by the American Physical Therapy Association or theCommittee on Allied Health Education and Accreditation of theAmerican Medical Association, as indicated by school transcript ordiploma.
• Ability to work independently and manage timeeffectively.
• Strong interpersonal, organizational, and problemsolving skills.
