• Will be doinghome visits about 25% of the time.

(This will vary from week toweek.)

REQUIRED

• Bachelor’s degree in physical therapy
• Completion of a physical therapy program accredited by the AmericanPhysical Therapy Association (APTA), the Committee on Allied HealthEducation and Accreditation of the American Medical Association andthe American Physical Therapy Association or other equivalentorganization approved by CMS
• Active Physical Therapist licenseissued by state in which practicing.
• A minimum of one yearexperience working with the frail or elderly

PREFERRED

• 2 years asa practicing PT
• Master’s Degree (MSPT) or Doctor of PhysicalTherapy (DPT)
